Question Help with a Form in an email!

I would like to know how I can send a "Subscribe a Colleague" form in my newsletter email? If someone can give me the basic script from the <form> tag to the </form> tag I would be VERY appreciative!

I would also like it if someone would give me some feedback on my new site - http://www.straightfocus.com - I used to be Adamthewebguy which is at http://www.adamthewebguy.com but now that gets forwarded to straightfocus.com

Ok, here is what you do:

You have the script on your homepage already, upload this page and mark down the web address.

once you have that hosted on a seperate page, use the "insert Hyperlink" option in your e-mail program to link that page on the bottom of your e-mail's
