Default TWAIN interface Stylus app?

Does anyone know where I can find an application that can be called from TWAIN that allows you to draw using a Stylus then "Save" and have the image transfer using TWAIN into another software.

Kinda like using a TWAIN compatible scanner yet the data or scan is your handwriting.

I have a tablet PC and and a medical records software that can capture from TWAIN. I want to be able to capture from TWAIN in my medical records software - have it bring up the drawing software where I can write notes then save and have it transfer.

Is there a way to assign a program to be called as a TWAIN device?

Adobe Acrobat has this function, but it costs a bit of money.

I could try CutePDF, it's free, and has the handwriting feature:
