Default Website Slider Bar?

I'm creating a survey using HTML/PHP/MySQL for my job, but it needs to have a slider bar that the user can slide back and forth to select the strength of their opinion. I'm not sure how to program one...does anyone have any good examples or websites with a simple one that will work for this? I was thinking Javascript would be the only way to do this and I searched for a while, but I didn't find anything great.

You could also use a simple little flash scripting. I believe CSS could pull something like that off as well with some good coding. That above posted control is really cool though!

Thanks for the suggestions. I saw the softcomplex.com slider before I posted and didn't like the look of it at first, but I ended up using it Everything worked out fine and it was pretty simple to use too.
