this is long i dream to write page with html or anything else that work for me as feedback page but it's just doesn't work,

i tried many page with reediting them but it's seems there is some sort of problem with them,

can anyone help me to write one Feedback or contact page which contain the form that have the visitor's informaiton such as e-mail and the reset that after submit the information to my e-mail address or any other place took the visitor to other page that says, your post have been sent..!
