One nice thing about owning an iPhone that hasn't been mentioned yet is the customer service that accompanies it. If there's ever something wrong with your iPhone all you need to do is visit the apple store and they'll most likely give you a brand new phone for free or quickly fix your problem...
It isn't that big of a deal. I have an iPhone 4.. Yeah I did see the bars go down before I got a case.. No I didn't drop any calls. Now that I have a case the reception in exemplary.

If you're trying to reformat.
Unplug the all the other hard drives and only plug in the drive upon which you want to install the operating system.
Then systematically plug each other drive in and see which one is causing the problem.
Yeah, I removed the hard-drive and attached it too my main rig and started a check disk before I left for work. I'll see what the results are when I get home in a few hours.

Sounds like your hard drive is going to me honestly; however it could be any number of different things.. Have your run he memtest and HD test like mindover told you to?
This virus has been going around. You can kill it in safe mode. Just boot into safemode and take care of it with antivirus. It's a really simple virus to take down. I've taken it off several computers in the past weeks.
